Those who want real pvp play genres that focus on it. Games like Diablo were always pve oriented and the majority of players have been happy with that. PvP in itself is extremely hard to balance due to the lengths of possibilities that a character can hold, so generally devs choose a path pretty early in the development to define the main focus of their game (and manage their time and resources accordingly).
